I give the Institute of Theology and Ministry Training authorization to make charges to my card in the amount
stated on the day of the month specified for the period of my program. As stated in the Financial Agreement of
my application I understand that I am personally obligated to pay tuition for my program and if I do not pay
tuition according to the plan I have chosen (Installments) the Institute reserves the right to forward my account
to a collection agency for payment which could reflect on my credit report. I also understand that the Institute
may withhold subject materials and take away access into the on-line classroom (the Institute's Website). I will
notify the Institute of any and all changes relative to this agreement before transactions are scheduled to be
made. I acknowledge that failure to pay tuition as scheduled will result in a “forfeit of Scholarship(s)” which
will then require that I pay full tuition. Further, the Institute has the right to dismiss me from my program and
withhold conference of my credentials.
